Report from Patient List

1. Sign into MySOZO as a clinician
2. From the homepage, search for the patient whose report you wish to view or print
3. Select “Report” button

Please Note: Use this option only if you want to print out the latest result.

Create Report Pop-up

1. Select the type of assessment to be printed
2. Select the history graphs to be printed
3. Set date range for history charts or select “All”
4. Select “Create”
